The top three plans by enrollment tell you a lot about how Louisville chooses: Anthem Medicare Advantage (HMO-POS) leads with over 10,200 enrollees, followed by Humana Community (HMO) at 6,400 and Humana Gold Plus at nearly 4,900. Anthem and Humana dominate this market, but that doesn't automatically make them the right choice for you.

Networks

Hospital Networks Matter in Louisville

One of the biggest factors in choosing a Medicare Advantage plan here is which hospitals are in-network. Louisville's major systems include:

Baptist Health Louisville (519 beds, 3-star CMS rating): the largest private hospital in the metro

UofL Health / University of Louisville Hospital (404 beds): the region's academic medical center with 1,200+ providers

Norton Healthcare: multiple locations across the metro, including Norton Clark Hospital in Jeffersonville

Jewish Hospital (part of UofL Health): 1,261 beds, a major downtown facility

If your doctors are in the Baptist Health system, you need to pay close attention. Baptist Health ended Medicare Advantage contracts with Humana, UnitedHealthcare, and Wellcare in recent years over reimbursement disputes. That means some of Louisville's most popular MA plans may not cover Baptist providers as in-network. We track these network changes and will flag any conflicts before you enroll.

HMO plans (32 available in Jefferson County) require in-network care and referrals. PPO plans (35 available) give you out-of-network flexibility at higher cost. We'll help you figure out which structure actually works for how you use healthcare.

Coverage Options

Medicare Supplement (Medigap) in Louisville

If you prefer the freedom of Original Medicare with nationwide doctor acceptance, a Medigap plan fills in the coverage gaps. In Kentucky, the major Supplement carriers include Humana, Aetna, Anthem Blue Cross Blue Shield, UnitedHealthcare, and Mutual of Omaha.

Plan G is the most popular option for anyone who became Medicare-eligible after January 1, 2020. You pay only the annual Part B deductible ($257 in 2025), and the plan covers everything else. Kentucky uses attained-age pricing for Medigap, meaning your premium increases as you age, so locking in a plan early matters.

Kentucky also has two Medigap protections worth knowing about:

Every carrier must offer all of its Medigap plans to under-65 disability and ESRD beneficiaries at no more than the weighted average of 65+ rates

60-day birthday rule:

You can switch Medigap carriers on the same plan letter within 60 days of your birthday without medical underwriting
