Understanding Your Health Insurance Options

ACA Marketplace Plans (Kynect / Healthcare.gov)

Marketplace plans are the most common option for individuals and families under 65. Offered through Kentucky’s state-run exchange (Kynect) or the federal Healthcare.gov marketplace in Ohio, these plans cover pre-existing conditions and may qualify for significant premium subsidies based on your income.

ACA plans typically use HMO networks, meaning you’ll have a primary care physician and need referrals for specialists. If you qualify for a subsidy, an ACA plan can be one of the most affordable options available. MedMyWay will run your numbers and show you exactly what you’d pay based on your household income.

Non-Marketplace PPO Plans

If you’re looking for more flexibility in choosing doctors and specialists — or if you don’t qualify for ACA subsidies — a non-Marketplace PPO plan may be a better fit. PPO plans offer larger national networks, no referral requirements, and often more predictable out-of-pocket costs.

Short-Term Health Insurance

Short-term health plans are designed for gaps in coverage — between jobs, after aging off a parent’s plan, or while waiting for Medicare eligibility. UnitedHealthcare and Allstate both offer short-term PPO plans available in Kentucky and Ohio.

Keep in mind that short-term plans don’t cover pre-existing conditions and don’t meet ACA requirements. They’re a temporary solution, not a long-term replacement for comprehensive coverage. MedMyWay will help you understand whether a short-term plan makes sense for your situation.

Medicaid & CHIP

If your income falls below a certain threshold, you may qualify for Medicaid (called Medicaid in Ohio or through Kentucky’s Medicaid program) or CHIP for your children at little to no cost. MedMyWay can help you determine if you qualify and point you in the right direction for enrollment.

When Can You Enroll in Health Insurance?

Unlike Medicare, individual health insurance under 65 has specific enrollment windows:

  • Open Enrollment: November 1 – January 15 each year. Anyone can enroll in or switch ACA Marketplace plans during this period.
  • Special Enrollment Periods (SEPs): Triggered by qualifying life events like losing job-based coverage, getting married, having a baby, or moving. You typically have 60 days from the event to enroll.
  • Medicaid & CHIP: Open enrollment year-round — you can apply any time if you qualify based on income.

Non-Marketplace PPO and short-term plans are generally available year-round regardless of enrollment periods.

Frequently Asked Questions About Health Insurance Under 65

How do I know if I qualify for a subsidy on an ACA plan?

ACA subsidies are based on your household income relative to the federal poverty level. Generally, individuals earning between 100% and 400% of the federal poverty level qualify for premium tax credits. In some cases, subsidies are available above that threshold. Can I get health insurance if I have a pre-existing condition?

Yes — ACA Marketplace plans are required by law to cover pre-existing conditions and cannot charge you more because of them. Non-Marketplace PPO and short-term plans may have different rules, which is why working with an independent agent to find the right plan type is important.

What's the difference between an HMO and a PPO?

HMO plans (common in ACA Marketplace) require you to choose a primary care physician and get referrals to see specialists. PPO plans allow you to see any in-network provider without a referral and often cover out-of-network care at a higher cost. PPOs offer more flexibility but typically come with higher premiums.
